Basic documents including: - claim form, - travelling schedule, - airline ticket, - boarding pass and - a copy of passport/ Identity card of claimant
Medical report and original Medical receipt/bills
Travel deposit receipt and airlines written confirmation on the reason for disruption/ interruption to the trip
Hotel accommodation confirmation advice
Police report
Purchase invoices for items claimed
Rental vehicle receipt and agreement, documentary proof of accident and excess receipt/repair invoice
Death Certificate and Letter of Administration or Grant of Probate (in the event of Death)
Correspondence from third party (when other parties making a claim against you)
All other documents which can facilitate the consideration of claim
